*Details needed for entry permits and other travel formalities
*Date of birth for each participant: dd/mm/yyyy
*Place of birth (city and country) - for each participant
*Address/City/Zipcode/Country for each participant
*Mobile Phone Number for each particpant
*E-mail address for each participant(s)
*Type of preferred accomodation (Dbl(Twn beds/Single) - subject to availability
*Comments - pls feel free to comment, add information (e.g. about health & meals) and eventual questions. Pls feel free - we are happy to assist
*Deposit Information: A deposit for each participant is requested and will reserve a place for you on this Sovietistan Travel Trip (usually 25% of total tour fare. When Reservation Form is received, we reply about payment information will be mailed. Your booking will be confirmed after receipt of your payment. Full payment of the trip is due 120 days prior to departure date, unless mentioned otherwise.
